Forms and Documents

To apply for your license (by mail or in-person) you must present the Coast Guard with certain documents. The following is to be used as a guide. Be sure to visit the Coast Guard site at http://www.uscg.mil/nmc/applications_forms.asp to be sure you use the most current version of each respective form.


To be turned in to the USCG for the issuance of your license:

Course-issued Diploma (Issued by us upon successful completion of exams)

License Application (License Application)

Small Vessel Sea Service Form (Sea Service Form)

Medical Evaluation Report (Medical Form) - to be filled our by your Doctor

Copy of your TWIC Card or proof you applied (TWIC Application Information)

Character References (Character Reference Form)

Proof you've taken a USCG Approved Drug test (get the forms from us)

When your package is put together, you can appear in person (with an appointment) at your local REC (Regional Exam Center) or simply send in your packet by mail.
